The Foundation for a Healthy Kentucky is seeking far-reaching input from Kentuckians to help in developing our plan of work for 2012-2017.
The 2011 Community Survey seeks your views – and those of your neighbors and colleagues – on the most important health issues facing Kentucky and what you believe the Foundation can do to address them. Your views will help shape our work over the next five years.
The estimated time to complete this survey is less than 15 minutes. It is voluntary and all individual responses are confidential.
